Histopathologic examination 1)
MACROSCOPY: Submitted are two vials with skin punch biopsies with a diameter of 0.5 cm. HISTOLOGY: 1.) Left carpus: The epidermis is intact. The dermis and adnexal structures are inconspicuous. Within the subcutis, there is an accumulation of fibre-rich connective tissue. The cells form bundles and occasionally walls. Mitotic figures are not detected. The cells are monomorphic with a chromatin-rich small elongated nucleus and a large amount of cytoplasm. 2.) Right knee: The epidermis is moderately hyperplastic with orthokeratotic hyperkeratosis. The hair follicles are dilated. In the deep dermis, there is an accumulation of adnexal structures containing hair follicles and hyperplastic sebaceous glands. Within the hair follicle lumina degenerate neutrophilic granulocytes are present. Hyperkeratosis is also evident. Lymphoplasmocytic infiltrates are detected in the adjacent tissue. Dense fibrosis is present between the adnexal structures. The process appears well-circumscribed. DIAGNOSIS: 1.) Suspicion fibroma. 2.) Fibroadnexal dysplasia. BEHAVIOUR: Both benign. PROGNOSIS: Favourable to guarded according to number of lesions. COMMENT: The histology revealed different processes as origin for the nodules. At the carpus mature fibre-rich tissue was detected. This is suspicious of fibroma. Fibromas are benign neoplasms originiating from connective tissue. Complete excision is curative. Differential diagnosis might be a scar formation. In this case usually however involvement of the overlying epidermis is evident which was not the case in this material. If there are several nodules, nodular dermatofibrosis should be taken into consideration. This rare disease occurs in combination with renal neoplasms. Fibroadnexal hamartoma (fibroadnexal dysplasia) is a non-neoplastic benign proliferation of distorted adnexa, fat, and collagen. This process most often forms discrete nodules on the distal extremities of dogs. Leakage of keratin or other debris into the adjacent dermis may evoke a secondary, usually sterile, inflammatory response. Clinical behavior is benign and complete excision is typically curative. Digital lesions may produce extreme disfiguration and may require amputation.
