"CYSTINURIA DIET :
Dietary modification, with the accompanying alkalinization of the urine, is the approach we recommend. The intent of this approach is to reduce the likelihood of the stones forming in the first place. This way, there is no need to surgically remove or medically shrink anything! It may seem that if we simply avoided feeding anything with cystine in it to the animal, that would solve the problem. Afterall, if there were no cystine, we would not have to worry about it being reabsorbed. Unfortunately, that is not the case. Cystine is not the “enemy”, in fact, it is an essential amino acid, meaning it must be a part of the diet of a healthy individual. It is so necessary, that the body will actually manufacture cystine out of other amino acids (methionine) if the diet is deficient in cystine. So, we will always have to be concerned with how best to rid the body of the cystine that cannot be reabsorbed. Of course it also makes sense that feeding the lowest cystine-containing sources of foods is a smart thing to do; we just cannot expect to eliminate cystine from the diet completely.

En dan de schone taak om uit te vinden welke voedingsmiddelen hoog aan cystine zijn en welke laag... Ja helaas, het is allemaal niet zo eenvoudig.
GOAL OF THIS DIET
There are many types of urine stones that can develop in people and animals. Cystine stones behave differently from other types of urinary stones (for example, struvite) in that they tend to form when their environment (the urine) is acidic (low pH). Therefore, our goal in modifying the diet of an affected individual is to alkalinize, or raise, the pH of the urine. This creates an environment where the excess cystine will be less likely to form into stones, and more likely to be excreted as part of the urine solution.

How do we control the pH of the urine? Well, the pH is largely influenced by the diet consumed. Generally speaking, high-protein diets, based on large quantities of meats, lead to acidic urine. Low-protein diets, based more on plant materials, lead to alkaline urine. This diet is designed to balance the proportions of meat and vegetables, such that the dogs’ requirement for protein is met, but not vastly exceeded. We have also carefully chosen the included food items based on their analyzed cystine and methionine content, selecting those with the least amount of these amino acids.

Another way to trick the urine into becoming more alkaline is to use what are called buffering agents. These are substances that will react in the digestive system and internal organs, to de-acidify, or “buffer” the urine components. This results in maintaining the pH at a higher level, thereby preventing stone formation. This diet offers the option of mixing some buffering agents into the food. The most common one is plain old baking soda (sodium bicarbonate). Some of the other recommended supplements, such as special forms of vitamin C, also serve to help control the urine pH.

Stones composed almost entirely of cystine form in dogs that have a renal tubular amino acid reabsorption defect known as cystinuria. Healthy dogs demonstrate 97% fractional reabsorption of cystine, while affected dogs excrete a much greater proportion of the filtered cystine load and may even exhibit net cystine secretion. Cystine is a relatively insoluble amino acid; therefore, in high concentration it may precipitate and form stones. Despite excessive urinary loss of cystine in cystinuric dogs, plasma cystine levels remain the same as in healthy dogs; in fact, the only morbidity or mortality associated with the inherited defect of cystine reabsorption is the sequela of urolith formation. Identification of cystine crystals by urinalysis indicates the dog is at risk of forming cystine uroliths. For poorly understood reasons, not all cystinuric dogs develop uroliths. However, the absence of uroliths does not preclude their future development, and preventive measures are indicated.

Dit lijkt erg op het probleem van de Dalmaat, maar het is toch anders. Bij de Dalmaat geldt dat hij geen purine kan reabsorberen door een enzym tekort/probleem in de lever. Dat geldt dus bij deze honden niet. De behandeling van Cystine lijkt alleen wel erg op de uraatproblemen van de Dalmaat (bij de Dalmaat heet het gruis/steen dus uraat).
Cystinuria is thought to be inherited as a sex-linked trait. However, in Newfoundlands it is transmitted as a simple autosomal recessive trait. The defect has also been reported in Dachshunds, Basset Hounds, English Bulldogs, Chihuahuas, Yorkshire Terriers, Irish Terriers, and mixed-breed dogs. Except for Newfoundlands, cystinuria has been recognized almost exclusively in male dogs. A urine cystine concentration of >75 mg/g creatinine in nonfasted dogs is predictive of susceptibility to cystine urolithiasis.

Cystine solubility depends on urine pH, with solubility increasing rapidly when urinary pH is >7.5. Dogs fed meat-based diets tend to excrete acidic urine, which leads to urine cystine supersaturation.

Honden die veel vlees eten: krijgen een zure urine: waardoor ze (als ze daar gevoelig voor zijn): cystinegruis/stenen krijgen. Ook dit lijkt weer erg op de Dalmaat. Een Dalmaat mag ook geen te zure urine krijgen en ook niet teveel vlees. Alleen krijgt de Dalmaat dus uraat.
Cystinuria is a lifelong defect of tubular reabsorption and cannot be cured. Cystine stones tend to recur within 1 yr without management to prevent recurrence, and they often recur despite attempts at prevention.
Dissolution and Prevention Protocols: Urinary cystine output should be reduced. Protein-restricted alkalinizing diets have been associated with reducing the size of cystine urocystoliths. Urinary cystine concentration can also be reduced by administering N-(2-mercaptoproprionyl)-glycine (2-MPG, tiopronin) or penicillamine. 2-MPG should be given at 15-20 mg/kg, PO, bid, for dissolution, and at 10-15 mg/kg, PO, bid, for prevention. Penicillamine (15 mg/kg, PO, bid) can be substituted for 2-MPG; unfortunately, ~40% of dogs treated with penicillamine exhibit anorexia and vomiting. The vomiting may be partially resolved by giving the medication with meals; however, a severe reduction in dosage or complete withdrawal is often necessary.

The urine should be alkalinized to a pH >7.5. Sodium bicarbonate added to the diet at 1 g (1/4 tsp)/5 kg, tid, readily accomplishes this, but because sodium supplementation may enhance cystinuria, potassium citrate (20-75 mg/kg, PO, bid) is preferred.

Urine volume can be increased by mixing water with the food. Salt should not be added to the diet because increased sodium excretion may cause increased cystine excretion. Provided urine volume is adequate and the urine pH is maintained above 7.5, most cystinuric dogs will pass urine that is only slightly supersaturated or undersaturated for cystine. Under such conditions, only relatively small doses of 2-MPG or penicillamine may be necessary to achieve 24-hr undersaturation"
